you cant leech:

-undead
-physical inmunes
-stone skins
* sometimes some demons seems like unleechable, example: torture demons in wsk. (guess is because of some sort of high physical resistance? dunno)
*some others requires amplify like mephisto (again for its high phys resistance i believe)

torture demons are unleechable due to some mythology of soul drain or something. I dont remember it exactly. They are designed that way though. Prime evils have a leech penalty in norm and nm. They are unleechable in hell iirc. I dont remember exactly, i dont play much in the way of melees with leech.
undeads cant be leeched without life tap. phys immunes cant be leeched at all unless amp breaks it but its very very low leech (due to high phys res still). stone skin can but since the phys res will be so high its not noticable without amp.

Wolfs - I know that already...
DoD - Just hitting on a fallen, I don't see my life go up.

A normal monster, Demon, non-undead, non-phys immune.

Could my barb be bugged or something?

I do 16K dmg, on a lets say 65% phys immune monster, so that leaves me to deal 5600 dmg. Since leech is 1/3rd in hell (from what i've heard) I would leech roughly 25% (with 74-75% leech) so 25% of 5600 would be 1400? dunno if i'm calculating this, but even if I did leech 400-500 per hit, my life still doesn't go up. I'm confused lol.

There is something called Drain Effectiveness that determines how much you can leech from a monster. Things in Hell often have about a 50% DE, although I didn't think that a fallen had that high of a DE. That will reduce your number as well. You can find the DE values in the monsters table if you want to do the math.

Are you watching your life bulb or do you have the character screen open to watch the numbers exactly? The bulb can be deceptive.

Have my char screen open and watching my life not go up. It goes up but thats my low repl life in effect. There is something seriously wrong with this char imo. I will be changing his build to better tank, with this I should have about 85% Life leech, unless i get my rings/amy my leech should be over 100%, then I can start testing better.

if the fallen has 5 life, and you leach 75% of that back, it might appear that you aren't getting any leech, are you sure this is the case (ll being broken for you)? mayhap you should try fighting a tougher monster

I don't get it. People have done stupider things and it was a valid question.
