This happens from time to time. I don't know how to replicate it consistently.
It also happens with Anya. I've even seen it happen with the soldiers in act 5.
It happened on bnet too.

It happens really rarely. I've witnessed this two times on vanilla. And yes, in Diablo 2 you can see new things even if you've played the game since it's launch.
